All accounts on server are getting this in the header:
MailScanner: Not scanned: please contact your Internet E-Mail Service Provider for details
Just started happening out-of-the-blue on one of our servers. Others are fine. We are running the following on all servers, with the latest version of the config server MailScanner package.
WHM 11.23.2 cPanel 11.23.6-R26881
REDHAT Enterprise 5.2 i686 on standard - WHM X v3.1.0
I have tried everything I can think of, including advice found here:

Also tried:
Force cPanel reinstall/update
Force MailScanner reinstall/update
Force clamV reinstall/update
Force MSFE reinstall/update
Force exim reinstall/update
Aldo tried:
touch /etc/exim_outgoing
/scripts/buildeximconf
/scripts/mailscannerupdate --force
service exim restart
Note:
I do get this when restarting via ssh, or via MailScanner addon utility in WHM (note "MailScanner:[FAILED]"):
Shutting down MailScanner daemons:
MailScanner:[FAILED]
Waiting for MailScanner to stop...
Starting MailScanner daemons:
MailScanner: [ OK ]
... but this is common on all other servers. MailScanner in WHM says MS is running. Still however, none of the mail is being scanned. "MailScanner: Not scanned: please contact your Internet E-Mail Service Provider for details"
Note, when doing a force update of exim, I noticed the following as one of the entries that came up in SSH:

Detected spam handling in acls, disabling spamassassin in routers & transports!.
SpamAssassin method remains unchanged

All accounts are being flooded with unfiltered spam. Anything else we can try?

We'd really need to see the results of a MailScanner debug run and any errors from the maillog when you try to start MailScanner, in order to determine what the problem might be. See step 7 in this FAQ for instructions on how to run MS in debug:
