Mail released from quarantine is caught again as spam. We haven't changed any configuration in a long time, this started happening with a newer MailScanner.

The postmaster address is in the whitelist, and the caught email shows the correct outgoing whitelisted postmaster address as the sender. The email is around 300K.

The email is caught for bad attachment name .BBC.PDF.

It's not being blocked for spam, it's being blocked for a potentially dangerous file attachment. The spam whitelist does not affect that.

If you want to allow double filename attachments you can modify the filename rules file. Please see this FAQ:
